Enrollment Checklist

Parents and students will need to bring the following:

o   Proof of Residence in Terry Sanford High School district: lease or buyer’s contract

o   Proof of Occupancy: major utility bill for current residence

o   Parent or Guardian Picture ID (State issued Driver’s license or Identification Card or Military Identification Card

o   Proof of legal custody if student is not with parents listed on birth certificate (Note: This must be an official court document.  A notarized statement does not signify legal custody.)

o   IEP or other exceptional children’s paperwork, if applicable

o   Complete Student Transcript (final 8^th grade report card for first time freshmen ONLY)

o   Student’s Social Security Card

o   Student’s Birth Certificate (original or certified copy) showing the parent(s)
name(s)

o   Official Immunization Record
